Alcohol Intensive Case Management (Wandsworth)

The Alcohol Intensive Case Management project targets severely alcohol dependent adults. They must demonstrate high levels of contact with acute health care services and also have a poor history of engaging with alcohol treatment services.

The service aims to:

- Reduce the severity and longer term consequences of severe alcohol dependency for adults who demonstrate a history of treatment resistant behaviour;
- Reduce the level of utilisation of acute health care services including a reduction in non-planned admissions;
- Increase levels of engagement with both primary health care and specialist alcohol treatment services.
- Provide a proactive and assertive approach to engagement.
- Offer flexible access (including treatment in the community where required).

Who is this service for?

Adult clients with:

- Severe alcohol dependence
- High levels of contact with acute health care services
- Poor history of engagement with alcohol treatment services

Who is eligible?

Clients need to be able to demonstrate a match against each of the following criteria:
1. Severely alcohol dependant adult
2. High levels of contact with acute health care services
3. Poor history of engagement with alcohol treatment services

In addition, clients must also demonstrate one or more of the following:
4. Presence of co-morbid illness
5. Socially isolated
6. High levels of social need (e.g. housing)

What do we offer?

• Comprehensive assessment of needs
• Direct support/assistance in engaging with services e.g. arranging and escorting clients to appointments
• Access to medically assisted withdrawal (in- or outpatient)
• Community case management/care assessment
• Single point of contact
• Referral to other specialist services (e.g. HPU, Social Services)
• Psychosocial interventions (e.g. CBT)
• Facilitated frequent contact: intensive, intermediate, ongoing
